Seishun Buta Yarou wa Yumemiru Shoujo no Yume wo Minai

Six months after Sakuta Azusagawa first met Mai Sakurajima in a library, the two are happily dating and the strange cases surrounding Adolescence Syndrome seem to have settled. That fragile peace breaks when Shouko Makinohara, Sakuta's mysterious first love, reappears as an adult.

Then Sakuta encounters another Shouko, this one still a middle school student living with a severe heart condition. The impossible coexistence of the two girls is somehow tied to Sakuta, whose old chest scars begin aching again whenever Shouko is near. With Mai caught in the middle and a young life at stake, Sakuta must confront feelings and choices for which there may be no painless answer.
